The student desiring admission in required to fill up a bio data form. On clearance from counselor of admission he/she will take the admission form from the office, fill up the form & get it signed by the admission authority & will deposit the fee.

All old students are required to fill up the admission form (old student) to provide up to date information & get it signed from coordinator before deposit the fee.

A student should carry his/her Identity card every day. Identity card is a must for every student as he/she may be asked to show it on demand as and when required. It is mandatory to carry the ID card during the clinical training as well as community health centers.
